Overview
In this peripatetic ACP role, you will deliver autonomous, expert care across East Cornwall’s community hospitals and urgent care settings, improving access and outcomes for patients with acute and complex needs.
You’ll work across SDEC, CATUs, Virtual Wards and Urgent Treatment Centres, providing advanced assessments, diagnostics and prescribing while leading and supporting teams.
The role supports NHS priorities through service development, education and quality improvement, ensuring high-quality, person-centred care wherever you are needed.
You will travel across multiple sites to optimise patient flow and care delivery.

Responsibilities
- Provide autonomous clinical care across Community Hospitals, SDEC, CATUs, Virtual Wards and Urgent Treatment Services
- Assess, diagnose, treat and manage patients with acute, chronic and complex conditions; manage admissions, investigations, prescribing and care planning
- Provide clinical leadership and support multidisciplinary teams; facilitate safe discharge planning; help reduce avoidable admissions
- Apply advanced decision-making to manage clinical risk, recognise deterioration and perform advanced procedures and IV therapies
- Advise, supervise colleagues and contribute to audit, research, service evaluation and quality improvement
- Support NHS priorities and service development through mentorship, education and high standards of practice and record keeping
- Travel across Liskeard, Launceston, Stratton, Bodmin and other East Cornwall settings to ensure expertise is available where needed
- Key requirements
- Current first level professional registration - RN Adult
- Independent Non-Medical Prescriber Qualification & registration with NMC
- Masters in Advanced Nurse Practitioner / verified portfolio or working towards
- Recognised teaching or mentorship qualification equivalent to level 3
- Knowledge of gerontology or a relevant senior medical specialty
- UK Registration (current)
- Strong analytical and judgement skills
- Ability to interpret complex situations and make decisions
- Clinical leadership and mentorship capabilities
- Advanced clinical practice skills (ECG interpretation, X-ray interpretation)
- Independent prescribing and pharmacology
