Overview
As an Advanced Clinical Practitioner at The Wooda Surgery, you will deliver safe, patient-centred care within a busy primary care team. You will assess, diagnose, treat, prescribe and appropriately refer patients with a range of acute and chronic conditions. The role includes wound care, diagnostic assessment, and infection control, working closely with the multidisciplinary team to improve outcomes. Opportunities exist to develop diabetes and respiratory care skills within the practice. This position is embedded in a supportive network that enables shared learning and growth.

Responsibilities
- Assess, diagnose, and treat patients presenting with acute and chronic conditions
- Prescribe medications and manage referrals
- Perform wound care management and diagnostic assessments
- Interpret diagnostic results and ensure infection prevention/control adherence
- Collaborate with the multidisciplinary team to improve patient outcomes
- Develop diabetes and respiratory disease management skills within the practice
- Work within the Torridge Primary Care Network to share experience and roles
- Provide patient-centred care and contribute to continuous improvement
Key requirements
- Current UK professional registration
- Experience as an Advanced Clinical Practitioner preferred
