As an Bank Advanced Clinical Practitioner, you'll be part of our valued team in our Hospital at Home service based in either Swindon or BaNES localities, providing acute hospital-level care to patients across the locality.

Main Responsibilities:
- Deliver a clinically autonomous service across Urgent Crisis Response, admission avoidance, frailty, and Virtual Ward (Hospital at Home) pathways.
- Using advanced clinical expertise, assess, diagnose, and manage patients with acute and complex needs.
- Apply advanced clinical reasoning to request and interpret investigations, initiate and review treatment plans, prescribe where appropriate, and monitor clinical and remote monitoring data.
- Work collaboratively within the multidisciplinary team to develop and review personalised care plans.
- Provide clinical leadership and act as a senior decision-maker and role model.
The Ideal Candidate:
- Registered with the NMC or HCPC.
- Accredited Advanced Clinical Practice qualification.
- Non-medical prescriber qualification.
- Significant experience in frailty, elderly care, community services, or acute care interfaces.
- Proven experience managing complex clinical caseloads and working autonomously at an advanced level.
- Demonstrable leadership and supervision experience within multidisciplinary teams.
- Clean driving licence and access to a vehicle for daily use.
