Overview
In this role you will provide expert clinical pharmacy input in the front door and admissions areas to support safe medicines use and discharge planning. You will work within a multidisciplinary team, using your independent prescribing qualification to optimise medicines and contribute to hospital-wide pharmacy services. The post emphasizes education, mentorship, and ongoing professional development on a pathway toward advanced practice. You will help lead service improvements and collaborate with colleagues to deliver high‑quality, patient‑centred care.
Responsibilities
- Provide expert clinical pharmacy input to front door and admissions patients
- Use independent prescribing to support medicines reconciliation, optimisation, and discharge planning
- Deliver broader clinical pharmacy services within the hospital
- Collaborate with medical,nursing and allied health professionals for seamless care
- Contribute to service development and quality improvement initiatives
- Mentor current and future pharmacy professionals and other healthcare staff
- Engage in education/training activities with Bangor University School of Pharmacy and supervise students and staff
Key requirements
- MPharm degree or equivalent
- Registered with the General Pharmaceutical Council
- Postgraduate MSc/Diploma in Clinical Pharmacy or equivalent experience/qualifications
- Broad post-registration experience
- Good verbal/written communication and teamwork abilities
- Evidence of change implementation and ability to meet deadlines
