Overview

In this role you deliver advanced clinical assessment and decision-making to patients in home and community settings, helping to prevent avoidable hospital admissions. You will work within an innovative multidisciplinary team across primary, secondary, community and social care to coordinate urgent and responsive care. You’ll lead autonomous clinical interventions and develop personalised care plans with GPs and allied professionals. This opportunity lets you shape community urgent care pathways and Hospital at Home initiatives while supporting patients to stay safely at home.

Responsibilities

  • Provide specialist triage, assessment and clinical intervention as an autonomous practitioner
  • Coordinate urgent community care across multiple interfaces (primary, secondary, community, social care)
  • Develop personalised care plans with GPs and other clinicians
  • Promote patient independence and self-management
  • Lead development, delivery and evaluation of evidence-based, patient-centred interventions
  • Support integrated working across community services and contribute to division goals

Key requirements

  • Advanced clinical skills
  • Autonomous practitioner with acute disease management experience
  • Ability to manage complexity and partnership working across organisations
  • Leadership and change management abilities
  • Excellent communication and IT skills
  • Current registration with NMC, HPC or other statutory body and independent non-medical prescriber (V300)
  • Substantial post-registration experience and independence in practice
  • Experience of teaching, mentoring and audit/research
  • Willingness to work across 7-day extended hours community service
  • Self-motivated and innovative
