Overview
In this ACP role, you will deliver high-quality, safe care within Hospital at Home and Urgent Community Response, collaborating with ACPs, nurse practitioners and GPs.
You will work across all four pillars of advanced clinical practice, making advanced clinical decisions and providing clinical leadership to support nursing and AHP teams.
The position emphasizes service development to enhance patient experience and enable cross-site work across University Hospitals Tees.

Responsibilities
- Deliver high-quality, safe patient care with advanced clinical decision-making
- Demonstrate all four pillars of the ACP framework (HEE 2017) in practice
- Provide clinical leadership and support to nursing/AHP teams to develop the service
- Lead on development of SOPs and local policies and participate in service improvement
- Collaborate across multiple sites and MDT to extend reach and impact
- Support junior colleagues to develop their scope of practice
- Engage in independent non-medical prescribing where legally permitted
- Maintain professional development portfolios and participate in ongoing assessments
- Travel between patients' homes to deliver care as required
- Contribute to audits and research within the service
- Key requirements
- Extensive clinical experience in the relevant specialties
- Experience leading audits and research within the service
- Ability to manage a highly complex caseload
- Capable of independent referral to MDT
- Experience developing SOPs and local policy
- Works within a supervision framework for ACP

- Experience leading a team
- Qualified MSc in ACP or equivalent, portfolio evidencing advanced clinical practice (HEE 2017)
