Overview
In this role you will deliver advanced clinical assessment and treatment in the community for adults in acute health or functional crisis, aiming to prevent hospital admissions and support timely discharge. You will lead clinical decision-making within a multidisciplinary team, provide expert autonomous practice, and contribute to service development and staff training. You will oversee a complex caseload, applying advanced skills to ensure safe, evidence-based care and effective reablement. This is a mission-driven, cross-functional position within CityCare’s Urgent Community Response, Virtual Ward and Reablement Team.
Responsibilities
- Conduct comprehensive assessments and deliver appropriate treatments for undifferentiated conditions to reduce hospital admissions
- Utilise Non-Medical Prescribing and/or Patient Group Directions per policy
- Order diagnostic tests when indicated and in line with guidelines
- Develop care plans with patients and carers, including safety netting and risk assessment
- Coordinate referrals and liaise with multi-disciplinary teams and external partners
- Provide clinical leadership and mentorship to primary care staff and students
- Lead service innovation, pilot new approaches, and bring new business into CityCare
- Maintain accurate records using organisational software and ensure data quality
- Support training and development opportunities across IRT/VW and wider community
- Demonstrate flexible, autonomous practice and participate in shift leadership and on-call duties
- Promote evidence-based practice and high standards of patient experience
- Adhere to professional codes of conduct and CityCare values
- Lead on governance, safeguarding and continuous professional development
Key requirements
- Registered healthcare professional (e.g., Nurse, NMC Part 1, ECP, Clinical Pharmacist, Physiotherapist)
- Recognised full Masters level qualification in Advanced Clinical Practice
- Independent Non-medical Prescriber (V300)
- Evidence of applying theory to practice
- Teaching/mentorship/supervision qualification
