Social Care Practitioner in Aberdeen

The post holder will be a clinically credible registered practitioner who is highly experienced and proficient in the management of the acutely unwell patient with a comprehensive background of experience in acute care. They will utilize their clinical expertise and specialist knowledge to deliver high quality evidence/research based care to the critically ill patient. They will review referred and ongoing patients and assess these patients who are considered "at risk" by the referring multi-professional team. They will act as a role model providing support, education and training to the multi-professional team caring for the critically ill patient. The post holder will have a degree of autonomy where they will be responsible for a caseload of patients across a number of specialties, to include an element of lone working. The post holder will be expected to use their experience to support the implementation of clinical and non-clinical governance and to modernise patient services. In addition, the post holder will be responsible for participating in clinical audit and nursing research. They will be required to work flexibly as a member of the ACT team to ensure standards are being maintained, evaluated and where necessary improved. In addition, to the obligatory advanced clinical skills, the practitioner may be required to develop a range of additional skills or techniques that fall outside the usual scope of nursing practice.

Main duties of the job

• Meet National Outreach Forum competencies and work within service specification for critical care services.
• Demonstrate the use of evidence-based specialist skills and practice in the care of critically ill patients, including the use of an effective communication tool (SBAR).
• Demonstrate the use of specialist knowledge and skills in the assessment, planning, implementation and evaluation of care delivered to the critically ill patient, ensuring the appropriate documentation is maintained and completed.
• Demonstrate the interpretation and delivery of appropriate action/treatment/therapy to the critically ill patient, ensuring records are updated and maintained. This includes the transfer of patient's intra-hospital.
• Monitor and evaluate the condition of patients using the National Early Warning Scoring system, recognising and alerting staff to changes in the patients' condition.
• Assist and participate in the follow up care of patients discharged from critical care.
• Negotiate with the ward multi-professional team to improve clinical care given to the "at risk" patient.

Person Specification

Training and Qualifications

• Essential: Current first level NMC registration or equivalent, Mentorship qualification, Medicines management competency, Acute Speciality qualification.
• Desirable: Degree or working towards, Evidence of CPD level 7, Leadership training, ALS, Non-Medical Prescribing qualification (essential for band 7 post), Assessment diagnostic qualification.

Experience

• Essential: Post registration experience, ideally within critical care or acute setting, Demonstration of evidence based practice, Evidence of ability to apply critical care skills outside of the critical care environment, Evidence of appropriate delegation and leadership skills, Understanding of change management, Recognition of own limitations.
• Desirable: Involvement in project work, Evidence of conflict management skills, Involvement in guideline development, Evidence of leadership or management skills, Ability to provide feedback, Proven track record of teaching experience.
