The post holder will conduct comprehensive Continuing Healthcare and Funded Nurse Care assessments for adults with complex health needs, ensuring timely assessment, review, and monitoring.
The post holder will ensure appropriate care management with colleagues and may present assessments for BSOL panel ratification.
Follow the National Framework for NHS Continuing Healthcare, including organising and coordinating MDT eligibility recommendations for ICB verification.
Support delivery and implementation of Personal Health Budgets (PHBs) and DOLS/LPS for the assigned caseload.
Ensure the clinical team maintains data systems per standard operating procedures.
Handle daily CHC Service inquiries and unexpected case management issues.
