We are looking for a compassionate, resilient, and self‑motivated individual to work with people on probation in Chesterfield. The role involves primary care in probation offices, helping clients improve health and wellbeing through ongoing support, behaviour change, and referrals to services.

Responsibilities

  • Meet with people on probation to improve their health and develop healthier lifestyles.
  • Manage Health Trainer champions.
  • Help build confidence, motivation and self‑efficacy among people on probation.
  • Support behaviour changes in areas such as diet, exercise, smoking, stress and mental wellbeing.
  • Help people find and access the health and wellbeing services they need.
  • Complete health and wellbeing assessments and devise tailored personal health plans.
  • Help people on probation identify how their way of life might affect their health and wellbeing and support them in making desired changes.
  • Assist people on probation to access social groups, healthcare, and other related appointments.
  • Work collaboratively with probation practitioners and other related services to provide wrap‑around support.
  • Manage a varied caseload and meet strict deadlines to ensure each person gets the support they need.
  • Deliver group workshops and drop‑in sessions as well as one‑to‑one support.
  • Maintain meticulous records of every interaction.

Qualifications and Requirements

  • Relationship building: genuine, non‑judgmental connection with people on probation, respecting confidentiality and boundaries.
  • Safeguarding awareness: identify safeguarding risks and follow appropriate processes.
  • IT and administrative skills: keep accurate records to measure client progress.
  • Resilience: stay committed and see things through.
  • Flexibility: adapt to frequent changes in referral flow.
  • Transport: travel between probation offices and community sites each day; reimbursement for travel beyond the base.
  • Eligible for an enhanced DBS check and HMPPS EL1 vetting.

Pre‑Employment Requirements

Due to the time required for an enhanced DBS check and HMPPS EL1 vetting, successful candidates will receive a start date at least six weeks from the point of offer.
