What you will do:

  • Conduct thorough needs assessments, develop care plans, and facilitate risk enablement, reviews, and safeguarding for adults.
  • Champion personalisation, promoting choice and control for service users to foster self-management of their needs.
  • Collaborate efficiently with multi-disciplinary teams, including Occupational Therapists, community officers, and health partners, to provide holistic support.
  • Act as a critical liaison in hospital discharge planning and community enablement, supporting coordination and advocacy for crucial services.
  • Maintain meticulous case records and electronic systems, preparing high-quality reports for continuous improvement.

Must have CHC or Hospital or Long Term Case experience. Must be able to work 2 days from office within city boundaries.
