What you will do:

  • Conduct investigations and perform structured assessments, producing detailed, evidence-based reports for complex situations.
  • Deliver continuous advice and support to individuals, families, and key workers, especially focusing on adults at risk.
  • Ensure that planning, agreements, and monitoring processes are established in cases, including safeguarding adults and mental health support.
  • Participate in devising preventative strategies and support services aimed at optimising patient outcomes.
  • Develop and maintain strong connections with various agencies within the hospital and the wider community.
  • Evaluate client needs and collaborate on strategies to address these requirements.
  • Offer guidance and information about local services and practices relevant to adult care.

Significant post-qualifying experience in a similar role within adult care. Extensive experience working within a fast-paced healthcare setting, ideally within an Integrated Discharge Team. Proficiency in undertaking safeguarding assessments and working within the frameworks of the Mental Capacity Act. Proven ability to deliver evidence in professional settings. Registered with Social Work England with a valid social work qualification (e.g. DipSW, CQSW, CSS, Degree in Social Work).
