• Conduct assessments and reviews for adults requiring care and support, working collaboratively with a multi-disciplinary team to ensure comprehensive care plans.
• Develop and maintain strong working relationships with clients, carers, and other professionals, ensuring effective communication and collaboration.
• Advocate for service users’ needs and rights, promoting their wellbeing and independence while safeguarding their vulnerabilities.
• Implement safeguarding procedures and report any concerns in line with local policies to protect adults at risk.
