What you will do:

– Conduct comprehensive assessments of adults with care and support needs, including risk assessments and capacity evaluations, to inform care planning and discharge arrangements.
– Undertake interventions that support adults, their families, and carers to achieve positive outcomes, ensuring the person is at the centre of all decision-making.
– Coordinate and manage packages of care, liaising with health, social care, and community services to facilitate timely and safe hospital discharge.
– Provide effective social work supervision and reflective practice within the team.
– Maintain accurate records and prepare reports, including for safeguarding and legal purposes.
– Participate in multidisciplinary team meetings and contribute to safeguarding adults’ procedures.
– Advise and support colleagues and partner agencies on complex cases, ensuring compliance with relevant legislation such as the Care Act 2014, Mental Capacity Act, and Mental Health Act.
– Promote strengths-based practice and uphold professional ethics and human rights frameworks.
– Engage in continuous professional development and contribute to service improvement initiatives.
