About the Role
You will promote the well‑being of adults and carers, facilitating independence and access to community resources. Your responsibilities include:
- Engaging in meaningful conversations with adults and carers to assess needs and develop personalized support plans.
- Collaborating with professionals and agencies to provide multi‑agency support, enabling individuals to live independently.
- Ensuring compliance with statutory requirements, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
- Participating in safeguarding procedures and interventions, utilising the Making Safeguarding Personal approach.
- Representing the council in court proceedings and advocating for the welfare of service users.
- Conducting assessments and support planning using a strength‑based approach.
- Collaborating with stakeholders to create sustainable care packages and prevent the need for statutory services.
- Maintaining accurate records and engaging in reflective practice and supervision.
- Promoting the welfare of children and reporting safeguarding concerns as necessary.
Requirements
Experience:
- Proven experience in social work, including assessment and support planning for individuals and caregivers.
- Experience in adult social care, especially with older adults and those with physical disabilities.
- Track record of involving users and caregivers in their care arrangements.
- Extensive experience collaborating with health and social care agencies and professionals.
- Proficiency in utilising community resources and networks to develop sustainable care plans.
- Evidence of Continuous Professional Development.
Knowledge, Skills & Abilities:
- Understanding of social work theories and practices.
- Familiarity with legislation pertaining to adults and caregivers.
- Proficiency in strength‑based practice.
- Clear and effective verbal and written communication.
