What You Will Do:

Supervise and manage a team of four Social Care Practitioners and one administrator.

Sign off and oversee Care Act Reviews and assessments for older adults, and individuals with learning disabilities and autism.

Support staff with mental capacity assessments, providing guidance and case direction.

Build connections with third sector organisations to enhance outcomes for service users.

Lead a one-day-a-week duty rota for the Learning Disability Service, managing new referrals, package queries, and safeguarding triage.

Drive targeted project work to improve reviewing outcomes across key service user groups.
