- ADDITIONAL HEALTH NEEDS: One client wears incontinence pants due to rectal and urethra prolapses. Neither client has a dementia diagnosis, but both experience memory difficulties. One client wears hearing aids and is unable to dress independently due to Parkinson's disease.
- FAMILY SITUATION: The clients have two adult sons with learning disabilities and one daughter living abroad. The family live at a distance but are very supportive and wish to help their parents remain in their family home for as long as possible. The client's spouse is becoming increasingly concerned about managing their partner's care as their own health needs grow.
