Muscular Dystrophy UK is seeking a Support Caseworker in Greater London to provide timely, compassionate, person-centred support to individuals and families referred to our services. You will develop personalised support plans and help people access information and services, while maintaining accurate records in line with GDPR and safeguarding policies.
Working with healthcare professionals and partner organisations, you will signpost to internal and external services, support progression.
