Moree Community Health is seeking a motivated and experienced Diabetes Educator and Cardiac Rehabilitation Clinical Nurse Specialist Grade 2 (CNS2) to join our multidisciplinary team. This is a rewarding community-based role offering the opportunity to provide high-quality, individualised, and case-managed care to clients with complex health needs, while contributing leadership to specialty clinical practice and service delivery.

In this position, you will deliver comprehensive Diabetes Management and Cardiac Rehabilitation services across both community and acute care settings within Moree Health Services. Outreach diabetes services are also provided to surrounding communities including Warialda, Bingara, Mungindi, Boggabilla, and Toomelah. You will play an active role in the Medical Outreach Indigenous Chronic Disease (MOICD) Program, working collaboratively with a Dietitian and Aboriginal Health Worker to improve health outcomes for Aboriginal and Torres Strait Islander clients living with diabetes. This role is ideal for a clinician who enjoys working autonomously, values collaborative practice, and is passionate about delivering culturally safe, client-centred care.

Key Responsibilities

  • Provide specialised, evidence-based Diabetes Management and Cardiac Rehabilitation care
  • Deliver individualised, case-managed care for clients with complex needs
  • Provide leadership and support for specialty clinical practice within the service
  • Participate in community outreach clinics and acute care support
  • Collaborate within multidisciplinary teams, including MOICD program partners
  • Contribute to service development, quality improvement, and education initiatives

About You

You are a motivated and autonomous clinician with strong skills in diabetes management and an interest in expanding your expertise in cardiac rehabilitation. You demonstrate excellent clinical judgement, communication skills, and a commitment to improving health outcomes in rural and community settings.

Essential / Desirable Criteria

  • Current registration as a Registered Nurse (AHPRA)
  • Postgraduate qualification in Diabetes Management and/or Chronic Care
  • Minimum 3 years postgraduate clinical experience
  • Experience working in community and/or rural health settings (desirable)
  • Commitment to culturally safe practice and collaborative care

Applicants with less experience in Diabetes Education will be considered if they have completed, or are willing to commit to completing, relevant postgraduate qualifications within 12 months of appointment. These applicants will be graded and remunerated according to qualifications at commencement and will progress to CNS2 classification once requirements are met.
