Join our team as a Community Dementia Advisor based at The Bridge, Tipton DY4 0SL. The role offers a salary of £21,307 per annum for 30 hours per week on a fixed term contract until 28/09/26 (subject to continued funding).
As the Community Dementia Advisor, you will be responsible for addressing the wellbeing and social support needs of people diagnosed with dementia and their family carers. Working as part of a team, you will focus on the strengths, interests, and talents of people with dementia to offer social opportunities and meaningful engagement in community activities.
Main duties of the job:
- Work with individuals living with dementia and their family carers, providing maintenance support and call care according to the service casework model.
- Collaborate with Dementia Navigators to facilitate interventions for people with dementia and their families requiring intensive support during periods of transition, complexity, and crisis.
- Recruit, deploy, and support volunteers to encourage participation in group activities and offer one-to-one befriending for individuals experiencing dementia.
- Lead initiatives to create dementia-friendly communities in the locality.

Job responsibilities:
- Promote the Sandwell Community Dementia Support Service within the local town, developing a network of support from health and social care professionals.
- Provide structured support to both the person with dementia and their carer for all allocated cases.
- Record actions undertaken and follow-up points within the shared CRM database (CharityLog).
- Co-develop wellbeing/support plans with individuals with dementia and their carers.
- Work collaboratively with Dementia Navigators to develop and deliver training.
- Ensure information provided to people experiencing dementia is easily understood and modified as necessary.
- Reach out to seldom heard groups and minimise barriers to accessing the service.
- Develop community networks to encourage attendance at arts and leisure activities.
- Signpost carers to local resources and act in an advocacy role.
- Build understanding of dementia and coping strategies for families.
- Act as a mentor and advocate to carers.
- Ensure monitoring and evaluation tools are accurately completed.
- Contribute to the continual improvement of the service.
- Attend regular staff meetings and training sessions.
- Safeguard clients using the reporting system.

Qualifications:
- Level 2 dementia qualification (or willingness to work towards it) or equivalent experience.
