Responsibilities:
- Actively participate in the delivery of the care and support as identified in the Individual Support Plan.
- Assist in the assessment and re‑assessment procedure for new and existing service users and clients.
- Personal hygiene requirements: bathing in bed, bathroom or other; care of pressure areas; looking after skin, teeth, mouth, hair and nails.
- Positioning and moving of service users and clients who are immobile; medication where covered in the Individual Support Plan.
- Assistance with toileting and the cleaning and emptying of commodes.
- Assisting with physical activity as needed, e.g., dressing, undressing, walking, sitting, passive exercises, etc.
- Position individuals correctly in bed and chair, referring at all times to the risk assessment for guidance.
- Report any unexpected inability or desire to eat or drink.
- Ensure that any significant changes to an individual's health and emotional condition are communicated to a line manager without delay.
- Support the Community Nurse (or other competent person) in delivering care and support to a dying individual, offering support to relatives and friends.
- Food collecting pensions or other benefits, if agreed on the Individual Support Plan.
- Help the individual maintain contact with family and friends.
- Maintain accurate records in respect of care and support given, tasks undertaken, monies transacted, etc.
- Participate in meetings and reviews where the worker’s direct knowledge of the individual's care is required.
