Join MT Consulting Group as a Discharge Coordinator. The role involves liaising internally with multidisciplinary teams and senior staff, as well as externally with Social Services, Primary Care Services, General Practitioners, Housing, Homeless Persons Unit, Department of Health, Borders Agency and Office of the Public Guardian.

Responsibilities:

  • Promote a smooth discharge process by completing discharge planning documentation.
  • Communicate with external agencies (Social Services, patient families and carers) and coordinate transport.
  • Liaise with multidisciplinary team members (occupational therapy, physiotherapy, speech and language therapy, dietitian, clinical psychologist and doctor) to ensure safe and timely discharge.
  • Provide access to patient homes to outside agencies and accompany patients to appointments.
  • Monitor and chase multidisciplinary team members and external agencies to ensure timely, safe discharge, optimizing bed usage and reducing length of stay.
  • Manage complex discharge issues.
  • Take responsibility for Discharge Notifications to Social Services and ensure reimbursement guidelines are observed.
