$1,800 Sign-on bonus payable over the first 6 months of employment!
The person in this position will be responsible for one-on-one support. We are looking for an individual who is comfortable providing employment support as well as individualized home support for this role.
- Responsible for providing one-on-one support to individuals receiving employment services.
- Implement individualized service plans as directed. Provide feedback to person served and document progress (to include case notes as well as electronic visit verification utilizing our online system).
- Provide job related counseling and support to assigned individuals at their job and in the community as needed.
- Develop and maintain effective working relationships with employers. Act as a resource for employers in job modification, reasonable accommodations, alternative communication systems.
- Responsible for assisting adults with disabilities maintain independence and improve daily living skills. Assist people to engage in meaningful activities both at home and in the community.
- Assist person served with community participation, health/safety/wellness, household management, and adaptive skills.
- The person in this position will be responsible for driving persons served both to/from work, and within the community. Locations of service include (but not limited to) Stillwater, St. Paul, Oakdale.
- Mileage reimbursement paid for mileage incurred providing transportation to persons served in the program.
Requirements
- Must relate well to people with disabilities.
- Must be qualified for employment in a 245D licensed program as determined by the MN Department of Human Services. Must pass other background checks as required by MSS and/or state and federal law.
- Mus secure and maintain current certification in First Aid, CPR, and Medication Administration.
- Must have and maintain a valid driver\’s license and meet MSS standards for providing transportation services. The person in this position may be responsible for transporting persons served in their personal vehicle as needed. Any employee driving an agency vehicle must obtain a Medical Examiner Certificate. Must have a clean driving record and be insurable under the conditions set by our insurance carrier. Must provide proof of auto insurance upon hire.
Benefits
- $1,800 sign on bonus paid over your first six months.
- Starting wage for this position is $19 – $19.50 per hour.
- Employees working 30 or more hours per week are eligible for a full benefits package including (but not limited to): company sponsored health & dental insurance, company paid life insurance & long term disability coverage, voluntary life insurance, voluntary short term disability insurance, voluntary vision, & accident insurance.
- Paid holidays, PTO, 401k eligibility after 6 months of employment.
