We are recruiting for Continuing Health Care Nurses in the Hoddesdon area to start ASAP. A variety of soft skills and experience may be required for the following role. Please ensure you check the overview below carefully.
Job Responsibilities:
- Must be able to complete Continuing Health Care Assessment and Decision Support Tool Assessments and a smartcard.
- The post holder will be responsible for facilitating multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures.
- The post holder would be responsible for completing Decision Support Tool assessments in line with the National Framework for NHS Continuing Health Care and Funded Nursing care.
- Ensure Continuing Healthcare guidance is followed and that services are delivered to maximise patient safety and understand the National Framework for NHS Continuing Healthcare and attend regular conferences to upskill knowledge and competencies.
- Be able to review care plans as per requests from patients and CHC verifiers to ensure that it continues to meet the patients’ needs whilst meeting a high standard of quality and safety.
- Liaise and update professionals and families who refer into Continuing Health Care to ensure the needs of the patient are met.
- To assess, review and plan an appropriate care pathway, in liaison with other healthcare professionals involved with the client.
- When appropriate assess clients for funded nursing care, ensuring timely reviews, and to engage with clients and their representatives to explain the assessment/review process.
- Will undertake statutory reviews and ensure that client's needs have been adequately met and progressed through the appropriate care pathway.
- To ensure all reviews, reassessments include appropriate reports and evidence to support the level of care need identified within the Health Needs Checklist / Decision Support Tool / Needs Portrayal process.
- As part of the review process, ensure a review of placement care package is also undertaken to ensure that all care needs are being met and to ensure best value for the funding authority.
- When appropriate proceed to the NHS - funded nursing care process.
- Responsible for collating complex evidence, and analysing the information, using the Decision Support Tool and liaising with the multidisciplinary team in order for a team recommendation to be made for eligibility for Continuing Healthcare funding.
