Older Adult Community Mental Health Nurse (Band 5) based in South Staffordshire.

Responsibilities

  • Deliver specialist assessments and evidence‑based interventions for older adults and dementia.
  • Manage a designated caseload, developing personalised care plans and risk assessments, and support service users to maintain community living.
  • Coordinate care and act as keyworker, working closely with mental health services, Staffordshire Police and other statutory/non‑statutory agencies.
  • Provide therapeutic interventions, supervise junior staff and deliver clinical and caseload supervision.
  • Make clinical judgments backed by reasoning, and work within the clinical pathway and service specification.
  • Travel across a large geographical area; must have access to suitable transport during work hours.
  • Adapt to unpredictable work patterns and challenging, distressing situations, exercising flexibility and compassion.

Qualifications

  • Essential: Qualified/registered mental health nurse (RMN) with evidence of professional updating and development in mental health services for older people; active on the NMC register.

Experience

  • Essential: Experience delivering therapeutic interventions to older adults and their carers; working with a range of statutory and non‑statutory agencies; involvement of service users and carers.
  • Desirable: Experience working therapeutically in community settings. Post‑registration experience in mental health.
