Support Worker MPA Recruitment is currently seeking compassionate, dedicated, and motivated Support Workers to join our team across a range of Supported Living Schemes, Housing with Care Facilities, and community-based services throughout the Derry/Londonderry, Strabane, and Limavady areas. As a Support Worker, you will provide person-centred care and support tailored to the individual needs of tenants. Working as part of a dedicated team, you will help promote independence, wellbeing, inclusion, and quality of life while ensuring the highest standards of care are maintained. Assist in the development, implementation, and review of individual support and care plans. Support tenants to develop and maintain independent living skills, including personal care, domestic tasks, social skills, and community engagement. Act as a key worker, providing ongoing support and guidance to assigned tenants. Monitor and report any changes in tenants' health, wellbeing, or behaviour. Participate in the planning and delivery of activities, outings, holidays, and community events. Complete support assessments, reviews, and outcome-focused support plans. Promote inclusion and support tenants to engage with their local communities. Ensure services are delivered in line with RQIA standards, Supporting People requirements, and organisational policies and procedures. QCF/NVQ Level 2 in Health & Social Care (or equivalent qualification). Minimum of 6 months' experience in a formal care or support role. Experience supporting individuals with learning disabilities and/or mental health needs. Minimum of 2 years' experience in a formal care or support role.
