Working collaboratively with internal teams and external partners, you’ll take a person-centred approach to empower individuals while helping to deliver key objectives within our Neighbourhood Strategy. We are looking for candidates that have a passion to advocate on behalf of people and communities.
What do our Wellbeing Officers do:
- Build strong, effective relationships with internal teams and external partners to support customers holistically.
- Assess at-risk customers and deliver targeted interventions, including referrals and signposting to appropriate support services.
- Manage and monitor cases, ensuring customers have all support in place.
- Provide proactive, person-centred support through home visits, crisis intervention, and multi-agency working.
- Contribute to service improvement and the Neighbourhood Strategy, using insight and case management to enhance customer outcomes.
What we are looking for:
- Good standard of education with minimum of 5 GCSE’s or equivalent at Grade C or above including Maths and English.
