We are seeking a talented and motivated Localities Lead for our transformational initiatives across hospital discharge, locality teams, and reablement services. Key Responsibilities: Lead and manage transformation projects aimed at improving hospital discharge processes and reablement services. Collaborate with local government departments, health services, and community organizations to create integrated and effective locality teams. Utilize strong analytical skills to assess existing processes, identify opportunities for improvement, and implement evidence-based solutions.
