Job Summary: The Advanced Clinical Practitioner (ACP) is a highly skilled healthcare professional who provides expert clinical care and leadership within the general practice. The ACP will work autonomously and collaboratively with the healthcare team to deliver high-quality, patient-centred care, focusing on assessment, diagnosis, treatment, and management of patients with acute and chronic conditions.

Qualifications
- Registered healthcare professional – e.g., nurse, paramedic, pharmacist – with current registration with the relevant professional body.
- Masters degree in Advanced Clinical Practice or equivalent qualification.
- Significant clinical experience in a relevant healthcare setting.
- Independent prescribing qualification (or willingness to obtain).
- Strong clinical assessment, diagnostic, and decision-making skills.
- Proficiency in using electronic health records and other digital tools to enhance patient care and communication.
- Excellent communication skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.
