Main duties of the job

Job Summary: The Advanced Clinical Practitioner (ACP) is a highly skilled healthcare professional who provides expert clinical care and leadership within the general practice. The ACP will work autonomously and collaboratively with the healthcare team to deliver high-quality, patient-centred care, focusing on assessment, diagnosis, treatment, and management of patients with acute and chronic conditions.

Key Responsibilities:

• Clinical Practice:
• Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic investigations.
• Diagnose and manage a wide range of acute and chronic medical conditions.
• Prescribe medications and treatments in accordance with practice guidelines and legal requirements.
• Provide expert clinical advice and support to patients, families, and carers.
• Leadership and Management:
• Lead and support clinical initiatives to improve patient care and practice performance.
• Mentor and supervise junior staff, including nurses and healthcare assistants.
• Participate in clinical governance activities, including audits, risk management, and quality improvement projects.
• Education and Training:
• Provide education and training to healthcare staff, promoting continuous professional development.
• Stay updated with the latest clinical guidelines, research, and best practices.
• Participate in personal and professional development activities to maintain and enhance clinical skills.
• Collaboration and Communication:
• Work collaboratively with GPs, nurses, and other healthcare professionals to ensure integrated and coordinated care.

Person Specification

Qualifications

• Registered healthcare professional – e.g., nurse, paramedic, pharmacist – with current registration with the relevant professional body.
• Masters degree in Advanced Clinical Practice or equivalent qualification.
• Significant clinical experience in a relevant healthcare setting.
• Independent prescribing qualification (or willingness to obtain).
• Strong clinical assessment, diagnostic, and decision-making skills.
• Proficiency in using electronic health records and other digital tools to enhance patient care and communication.
• Excellent communication skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.
• Strong interpersonal skills, demonstrating empathy, patience, and cultural competence.
• Experience in leading clinical teams or projects, with the ability to inspire and motivate colleagues.
• Skills in managing change and improving clinical services or pathways.
• Involvement in clinical research or quality improvement initiatives, with a focus on evidence-based practice.
