As a leader in hospice care, we focus on enhancing quality of life by offering physical, emotional, and spiritual support. We are currently seeking a skilled and compassionate Weekend Hospice Registered Nurse (RN) in the Harrisburg, Lancaster, and York, PA areas.
As a Weekend Hospice Registered Nurse, you will be responsible for managing and coordinating the care of patients in their homes, skilled nursing facilities, or assisted living environments from Friday 5pm to Monday at 7am. You will take call during these hours and perform patient visits (including admissions). You will work closely with an interdisciplinary team to assess patient needs, develop care plans, and provide direct nursing care to ensure the comfort and dignity of patients at the end of life. This position requires a nurse with clinical expertise, compassion, and a commitment to providing high-quality hospice care.
Key Responsibilities:- Taking call from Friday 5pm to Monday at 7am
- Provide skilled nursing care including medication management, pain control, symptom management, and emotional support.
- Act as a liaison between patients, families, and the interdisciplinary care team to ensure coordinated care.
- Educate patients and families on disease processes, medications, and end-of-life care options.
- Monitor patient progress and adjust care plans as needed to ensure comfort and dignity.
- Document patient care and progress in compliance with hospice regulations and company policies.
- Participate in regular team meetings to discuss patient status and plan of care.
- Current and unrestricted RN license in Pennsylvania
- At least 1 year of hospice or palliative care experience preferred
