Morley and District Primary Care Network have an exciting opportunity for a Health and Wellbeing Coach to join the team. This is a new and important role that will help shape and form our local healthcare offer in Morley, Leeds. The suitable candidate should be passionate about making a difference in primary care and enjoy working as part of a multi-disciplinary team across services. The role will involve working across the 6 practices we support, so the candidate must be able to be flexible and adapt to different businesses. These include: Windsor House Group Practice, Fountain Medical Centre, Leigh View Medical Practice, South Queen Street Medical Centre, Gildersome health centre or Drighlington Medical Centre.

Please note, Health and Wellbeing Coaches in PCNs must complete a Personalised Care Institute (PCI) accredited four-day health coaching training as the mandatory minimum standard. This training, which can be found through accredited training providers on the PCI site, should be completed prior to seeing patients.

Main duties of the job

Health and wellbeing coaches are non-clinical, personalised care roles. They work with people with physical and/or mental health conditions, people with long-term conditions, and those at risk of developing them. They focus on improving health-related outcomes by working with people to set personalised goals. They offer support to increase self-efficacy, motivation, and commitment to make changes to lifestyle and improve health. Health coaches are experts in behaviour change, working in a person-centred approach. The expectation of the role is to assist patients either face to face or on the phone (patient dependant) and involve assessing the patient in a holistic approach to manage long-term conditions through motivational coaching and behaviour change management. You will coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan. You will provide personalised support to individuals, their families, and carers to ensure they are active participants in their own healthcare; empowering them to take more control over their health and physical wellbeing, to live independently, and improve their health outcomes through joint care planning.

We are a 6 practice PCN with approximately 65,000 patients. We are rapidly developing our multi-disciplinary workforce, embedding our roles, and developing a team that makes a real difference to our patients and practices.

Job responsibilities

Referrals will be sent to you from other professionals of patients who could benefit from health and wellbeing coaching, it will then build you a case load of patients you will support over an agreed period of time. Health and wellbeing coaches will take an approach that considers the whole person in addressing existing issues and encourages proactive prevention of new and existing illnesses. They will take a non-judgemental approach based on strong communication and negotiation skills, which supports personal choice and positive risk taking, addresses potential consequences and ensures patients understand the accountability of their own decisions, making the patient at the centre of every conversation.

Person Specification

Qualifications

  • NVQ Level 3, Advanced level or equivalent qualifications or working towards
  • Training in motivational coaching and interviewing or equivalent experience

Experience

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)
  • Experience working in Primary Care
  • Experience of supporting people, their families and carers in a related role (including unpaid work)
  • Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity
  • Experience of working with the VCSE sector (in a paid or unpaid capacity), including with volunteers and small community groups
