We have an exciting opportunity in our Primary Care Network for a clinical pharmacist to join our team. The role will support in developing medicines optimisation services within our Network. Morley has a well-established PCN made up of 6 GP practices covering a population of 63,000. It is in a vibrant and busy market town 5 miles outside of Leeds. The role may be working across any site within our PCN.

Main duties of the job:

  • The successful candidate will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management.
  • Structured medication reviews.
  • Management of long-term conditions.
  • Management of medicines.
