We are looking for an experienced Practice Nurse who is able to demonstrate a high level of care working independently and alongside colleagues. This is a part time role, days and hours to be agreed with the need to hold a regular Saturday morning clinic.

About us
We serve approximately 11,500 patients in a semi-rural location and work from attractive well-appointed premises, where you will have your own consulting room. We are a multidisciplinary team consisting of 11 doctors, a GP trainee, pharmacists, nursing team, social prescribers, and a care navigation team. We also have an onsite pharmacy. We have exceptional clinical outcomes and strive to improve the health of our community and provide holistic patient experience.

Job responsibilities
To be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.
