About the Role
As a Housing & Support Coordinator, you'll support individuals to maintain their accommodation, promote independence and improve their wellbeing.
- Create and manage personalised support plans
- Provide housing and tenancy support
- Support clients with benefits, healthcare and employment
- Work with external agencies and partner organisations
- Maintain accurate records and contribute to service improvements
- Respond effectively to challenging situations and crises
What you'll need
- Experience in housing, supported housing or support services
- Strong communication and organisational skills
- Ability to work with vulnerable individuals and promote independent living
- A commitment to delivering person-centred support
