  • Develop and deliver holistic support plans that address mental health, emotional wellbeing, practical needs and housing management.
  • Support clients to manage their mental health and engage with appropriate treatment and care.
  • Support medication compliance, including prescription collection, repeat prescriptions and attendance at appointments where required.
  • Support clients to attend statutory and mental health-related appointments.
  • Motivate and coach clients to engage with appointments, activities and wider support services.
  • Identify, report and respond appropriately to safeguarding concerns, incidents and other risks.
  • Work alongside health, social care and other professionals to manage risk and escalations where necessary.
  • Provide housing-related support to help clients understand their tenancy rights and responsibilities.
  • Support clients with budgeting, bill management and other daily living skills.
  • Promote wellbeing, healthy living and access to appropriate community services.
  • Build effective relationships with health and social care partners to maximise client wellbeing and independence.
  • Support clients to access activities and opportunities that promote social inclusion and greater independence.
  • Maintain accurate and timely records of client contact, casework and outcomes.
  • Work collaboratively with colleagues and external agencies to ensure client needs are assessed and addressed appropriately.

What We’re Looking For

  • Experience supporting vulnerable adults with significant and complex needs.
  • Experience delivering client-centred support and managing a caseload.
  • Strong understanding of severe and enduring mental health needs and recovery-focused practice.
  • Experience completing needs assessments, support plans and risk assessments.
  • Good and current understanding of safeguarding and risk management.
  • Experience supporting individuals with budgeting, benefits, rent arrears and housing-related issues.
  • Understanding of the challenges associated with mental health, substance use, dual diagnosis, homelessness and social isolation.
  • Ability to build positive relationships and engage individuals with varying levels of need and risk.
  • Experience working collaboratively with health, social care and other external agencies.
  • Ability to work within a strengths-based and recovery-focused approach.
  • Strong organisational skills and the ability to work independently.
  • Good IT skills and the ability to maintain accurate case notes and database records.
