We are looking for a committed and reliable Outreach Worker to provide high-quality, person‑centred support to young people aged 16-17 and adults at risk within supported accommodation services. This is a flexible, temporary opportunity suited to someone with experience supporting young people, care leavers, vulnerable adults, or families. The role focuses on promoting independence, safeguarding wellbeing, and helping individuals achieve positive outcomes.

Key Responsibilities

  • Provide practical and emotional support to young people and adults at risk
  • Build positive, trusting, and professional relationships
  • Support individuals to develop independence and life skills, including budgeting, cooking, housing readiness, household management, and using public transport
  • Help individuals access education, training, employment, health services, community resources, and wider support
  • Advocate for individuals and ensure their views are heard
  • Contribute to support plans, risk assessments, statutory reviews, and reports
  • Keep accurate records of support, progress, incidents, and outcomes
  • Identify, record, and escalate safeguarding concerns appropriately
  • Liaise with social workers, personal advisers, housing teams, police, health services, and other agencies
  • Complete property health and safety checks with supported individuals
  • Attend team meetings, supervision, appraisals, and relevant training

About You

The ideal candidate will be compassionate, organised, resilient, and able to work flexibly. You will have strong communication skills and a clear commitment to safeguarding, equality, diversity, and inclusion.

Essential Requirements

  • Experience of working, volunteering, or relevant life experience with children, young people, adults at risk, or families
  • Understanding of safeguarding and child protection
  • Ability to build and maintain professional relationships
  • Strong communication and interpersonal skills
  • Good organisational and administration skills
  • Ability to manage a caseload and prioritise tasks
  • Ability to keep clear records and produce reports
  • Basic IT skills, including Microsoft Office
  • Ability to travel across the region by public transport or car
  • GCSE/O-Level English, or equivalent
  • A relevant Level 3 qualification in Health and Social Care, or equivalent experience, or willingness to work towards this

Safeguarding

This role involves working with children, young people, care leavers, and adults at risk. The successful applicant will be required to complete an Enhanced Disclosure and Barring Service check.
