We are recruiting for a Young Person Support Worker to join a dedicated service in Lewisham, supporting young people who have recently left the care system and are transitioning into independent living. This is a rewarding opportunity to make a genuine difference by helping young people develop the skills, confidence, and resilience needed to live independently and achieve positive outcomes.

Key Responsibilities

  • Manage and support a caseload of young people aged 16-25
  • Assist clients with developing independent living skills, including budgeting, cooking, tenancy management, and accessing education or employment
  • Complete support plans, risk assessments, and regular keywork sessions
  • Promote positive wellbeing, safeguarding, and personal development
  • Support young people to access benefits, healthcare, training, and community services
  • Maintain accurate case notes and ensure all records are up to date
  • Work in partnership with local authorities, social workers, and external agencies

Requirements

  • Previous experience supporting young people, care leavers, or vulnerable adults
  • Knowledge of safeguarding and risk management procedures
  • Experience completing support plans and maintaining case records
  • Strong communication and relationship-building skills
  • Ability to work independently and manage a varied caseload
