  • Build effective relationships with health and social care partners to maximise client wellbeing and independence
  • Support clients to access activities and opportunities that promote social inclusion and greater independence
  • Maintain accurate and timely records of client contact, casework and outcomes
  • Work collaboratively with colleagues and external agencies to ensure client needs are assessed and addressed appropriately

What We’re Looking For

  • Experience supporting vulnerable adults with significant and complex needs
  • Experience delivering client-centred support and managing a caseload
  • Strong understanding of severe and enduring mental health needs and recovery-focused practice
  • Experience completing needs assessments, support plans and risk assessments
  • Good and current understanding of safeguarding and risk management
  • Experience supporting individuals with budgeting, benefits, rent arrears and housing-related issues
  • Understanding of the challenges associated with mental health, substance use, dual diagnosis, homelessness and social isolation
  • Ability to build positive relationships and engage individuals with varying levels of need and risk
  • Experience working collaboratively with health, social care and other external agencies
  • Ability to work within a strengths-based and recovery-focused approach
  • Strong organisational skills and the ability to work independently
  • Good IT skills and the ability to maintain accurate case notes and database records
