We are looking for an organised, motivated, and enthusiastic Lead Outreach Worker to join our client’s Haringey based Outreach Team. This is an exciting opportunity to join our clients superb Outreach Service, assessing rough sleeper’s needs and assisting them to obtain temporary accommodation, linking them in with various avenues of support and delivering personalised case management along the way

To be a successful Lead Outreach Worker. You will

· Carry out Risk assessments with newly admitted rough sleepers

· Take responsibility to work with some of the more complex cases

· Create Support plans to aid everyone’s circumstances

· Possess an understanding of working with rough sleepers or vulnerable adults and supporting them to access services

· Have awareness of CHAIN or similar database

· A commitment to working in a challenging environment with the ability to be highly flexible throughout the working day

· Be able to organise your own diary, use your own initiative and possess high quality written and verbal communication

· Be able to guide others within the team who look to you s the experienced member of the team

To be a successful Lead Outreach Worker, you must have:

· Must have an in date Enhanced DBS check – Adults & Childs. If needed, please contact us.

· Confidence to manage a caseload of complex clients

· Excellent communication skills, and the ability to build strong effective working relationships.

· The ability to empower individuals to make positive changes and a willingness to develop on these skills.
