We are seeking an enthusiastic,self-motivated and well organised Home Visiting Practitioner to work 22.5 hours per week over 3 days withinour Primary Care Network Home Visiting Service. You will be a dedicatedprofessional who shares our values and passion about delivering high quality,joined-up clinical management of patients in their own homes.

Main duties of the job

You will have the clinicalexpertise and ability to autonomously assess and treat patients with a range ofconditions and be comfortable acting autonomously and seeing patients withoutprior contact with their GP. You will have experience in primary care workingand be confident in formulating rapid and accurate treatment plans.

Job responsibilities

Key Duties & Responsibilities:

  • To supportthe acute home visiting of patients from the practices that make up Mid Furness PCN
  • Undertakeadvanced assessment of house bound patients, resulting in an end to end episodeof care through treatment planning and prescribing (as required and appropriatewithin the scope of clinical competence and practice)
  • Arrangeclinical investigations, either directly or via the patients home practice
  • Arrangeonward referral to secondary care, community services and other PCN serviceseither directly or via the patients home practice
  • Gain insightand feedback directly with practices to ensure excellence in integrated servicedelivery
  • Develop andmake use of full scope of practice, including skills relating to independentprescribing.
  • Criticallyevaluate current research and demonstrate an understanding of the applicationof national and local guidelines and legislation relating to health and socialcare and their impact on service provision.
  • Beresponsible for and lead others in maintaining stock of equipment andmaterials, advising the correct procedures relating to the safe and competentuse, issue and retrieval.
  • Developrelationships and collaborative working approach across the PCN supporting theintegration of pathways in primary care.
  • Be accountablefor decisions and actions supported by a professional culture of peernetworking/review and engagement in evidence-based practice.
  • Activelyparticipate in the mentoring of colleagues and participate in the on-goingdevelopment of the role of paramedics and the function of home visiting
  • Implement allaspects of effective clinical governance for own practice, including regularaudit and evaluation, supervision and training.
  • Activelypursue development opportunities for practice and service delivery, policyimplementation and lead specific projects within specific area of practice
  • Maintain aprofessional portfolio for CPD recording learning outcomes throughparticipation in internal and external development opportunities and todemonstrate acquired highly specialist skills and knowledge of professionalpractice.
  • Duties willvary from time to time under the direction of the management leads, inagreement with the post holder, dependent on current and evolving practiceworkload and staffing levels.

Qualifications

  • Regulated Health Care professional with current registration
  • Willing and able to undertake Advanced Practitioner course if required (with appropriate qualifications in place to allow this)
  • Registered as a non-medical prescriber with the relevant regulator

Experience

  • Significant post registration experience where autonomous working at an advanced level has been acquired
  • Demonstrate working towards or willing to develop on commencement of post to academic Level 7 capability in paramedic areas of practice
  • Proven autonomous working with the ability to develop treatment plans independently
  • Experience of working within multi professional settings with consolidated clinical practice and contributing to effective team working
  • Experience of computer based clinical systems, (ideally EMIS Web)
  • Experience of clinical leadership
  • Proven track record of teaching and assessing
  • Experience of primary care specific advanced clinical skills
  • Experience of leading evidenced service improvement and innovation through service /practice development initiatives, audit or research
  • Team management

Other

  • Flexibility to work outside of core office hours if required
  • Understand the legal, ethical and professional responsibilities and accountability with regards to advanced level, autonomous practice.
  • Able to meet any travelling requirements of the post
  • Clear vision of role and commitment to working in Primary Care
