The predicted salary is between 36000 - 60000 £ per year.

We are seeking an enthusiastic, self-motivated and well-organised Home Visiting Practitioner to work 22.5 hours per week over 3 days within our Primary Care Network Home Visiting Service. You will be a dedicated professional who shares our values and passion about delivering high quality, joined-up clinical management of patients in their own homes.

Main duties of the job:

  • You will have the clinical expertise and ability to autonomously assess and treat patients with a range of conditions and be comfortable acting autonomously and seeing patients without prior contact with their GP.
  • You will have experience in primary care working and be confident in formulating rapid and accurate treatment plans.

Mid Furness Primary Care Network consists of 4 practices serving a population of 30,000 patients. Mid Furness PCN is a forward-thinking Primary Care Network. Our aim is to provide exemplary patient care; finding innovative solutions in general practice to deliver the best care we can to our patients.

Job responsibilities:

  • To support the acute home visiting of patients from the practices that make up Mid Furness PCN.
  • Undertake advanced assessment of housebound patients, resulting in an end-to-end episode of care through treatment planning and prescribing (as required and appropriate within the scope of clinical competence and practice).
  • Arrange clinical investigations, either directly or via the patients' home practice.
  • Arrange onward referral to secondary care, community services and other PCN services either directly or via the patients' home practice.
  • Gain insight and feedback directly with practices to ensure excellence in integrated service delivery.
  • Develop and make use of full scope of practice, including skills relating to independent prescribing.
  • Critically evaluate current research and demonstrate an understanding of the application of national and local guidelines and legislation relating to health and social care and their impact on service provision.
  • Be responsible for and lead others in maintaining stock of equipment and materials, advising the correct procedures relating to the safe and competent use, issue and retrieval.
  • Develop relationships and collaborative working approach across the PCN supporting the integration of pathways in primary care.
  • Be accountable for decisions and actions supported by a professional culture of peer networking/review and engagement in evidence-based practice.
  • Actively participate in the mentoring of colleagues and participate in the ongoing development of the role of paramedics and the function of home visiting.
  • Implement all aspects of effective clinical governance for own practice, including regular audit and evaluation, supervision and training.
  • Actively pursue development opportunities for practice and service delivery, policy implementation and lead specific projects within specific area of practice.
  • Maintain a professional portfolio for CPD recording learning outcomes through participation in internal and external development opportunities and to demonstrate acquired highly specialist skills and knowledge of professional practice.

Duties will vary from time to time under the direction of the management leads, in agreement with the post holder, dependent on current and evolving practice workload and staffing levels.

Qualifications:

  • Regulated Health Care professional with current registration.
  • Willing and able to undertake Advanced Practitioner course if required (with appropriate qualifications in place to allow this).
  • Registered as a non-medical prescriber with the relevant regulator.

Experience:

  • Significant post-registration experience where autonomous working at an advanced level has been acquired.
  • Demonstrate working towards or willing to develop on commencement of post to academic Level 7 capability in paramedic areas of practice.
  • Proven autonomous working with the ability to develop treatment plans independently.
  • Experience of working within multi-professional settings with consolidated clinical practice and contributing to effective team working.
  • Experience of computer-based clinical systems, (ideally EMIS Web).
  • Experience of clinical leadership.
  • Proven track record of teaching and assessing.
  • Experience of primary care specific advanced clinical skills.
  • Experience of leading evidenced service improvement and innovation through service/practice development initiatives, audit or research.

Other:

  • Flexibility to work outside of core office hours if required.
  • Understand the legal, ethical and professional responsibilities and accountability with regards to advanced level, autonomous practice.
  • Able to meet any travelling requirements of the post.
  • Clear vision of role and commitment to working in Primary Care.
