Job Description To be a role model for and support the delivery of competent and confident professional practice within a social work team.

To plan, support and co-ordinate LD packages and to take responsibility for managing complex situations including complex needs.

Responsibilities The effective management of cases, the delivery of cost-effective interventions and the achievement of all relevant statutory requirements and department policies, standards and guidelines.

To introduce and support the development of evidence based practice and to monitor and support provider organisation staff in the delivery of care and support.

To lead on the improvement of ICT systems on behalf of Practitioners.

To mentor and coach professional staff and peers including those less experienced and newly qualified social workers, ensuring a focus on evidence-informed practice.

To be responsible for practice audits within teams, working together with other advanced practitioners, consultant practitioners and the Performance and Quality Team.

To contribute to practice, policy and procedure improvements and developments through contribution of expert knowledge.
