Moorfields Eye Hospital NHS Foundation Trust in London is seeking a Learning Disability and Dementia Liaison Nurse to work with clinicians, safeguarding teams and carers to promote accessible care for patients with learning disabilities, autism and dementia.

You will support assessment, care planning and service navigation across network sites, provide expert advice, and help embed best practice in line with Care Act 2014, MCA 2005 and DoLS.
