The Eye Clinic Liaison Service bridges the gap between local hospital services and the community to provide service users who are partially sighted and severely sight impaired with critical emotional support, information and advice. This post will be based across the City Road site and the North West ECLO service covering Brent Cross, Northwick Park, Ealing and Potters Bar.
Responsibilities- Provide advice, information, practical and emotional support to adults of all ages affected by sight loss, and their families and carers.
- Act as a key contact person to support clients, family members and carers to navigate a variety of information and advice available and ensure they can access relevant support services at all stages of sight loss.
- Act as a key information and advisory link in the certification and registration process for blind and partially sighted people and promote the benefits of certification to eye clinic staff.
- Ensure safeguarding risks and regulations are understood, applied and managed in line with Moorfields Safeguarding policies.
- Empower patients so they can prioritise their own needs.
- Take up referrals to the service and assist with service provision and processing of referrals to the appropriate resource.
- Where needs are identified that are not specifically connected to an individual’s visual impairment, identify the route and signpost and refer on to other services.
- Develop excellent working relationships with internal clinical groups (Nurses, health care assistants, optometrists, consultants) and external departments (local authorities, sensory impairment, sight loss charities, community support groups).
- Attend team meetings, ECLO network meetings, training events and maintain own continuing professional development.
- Collate and maintain accurate client records, input quality data using common data collection tools and databases, and maintain confidentiality in line with data protection legislation.
- Ensure patient information resources and relevant literature are ordered, maintained and accessible.
- Maintain an understanding of relevant legislation and national policies in relation to adults with visual impairments.
- Provide advice to visually impaired people on welfare rights and entitlements and refer them to appropriate agencies.
- Provide general and specialist information on equipment and mobility training, and advise on adjusting and adapting depending on level of visual impairment.

Education/Qualifications
- ECLO Accreditation (or willing to work towards)
- Minimum 5 GCSEs or equivalent including English and Maths
Experience
- Previous experience of working in the field of vision impairment
- Experience of supporting emotional/distressed people
Knowledge
- An understanding of the needs of people living with sight loss
- Knowledge of charities/services available to support people with a visual impairment
- Knowledge of the benefits and concessions available to visually impaired people
