  • Undertake remote follow‑up consultations for patients prescribed CBPM, providing ongoing monitoring, review, and optimisation of treatment plans.
  • Recognise and respond to changing clinical needs, identifying deterioration and taking appropriate clinical action and escalation where required.
  • Independently prescribe CBPM in line with national guidelines, clinical governance frameworks, and organisational policies.
  • Work collaboratively with consultants and other clinicians within the MDT to ensure safe, evidence‑based prescribing and monitoring.
  • Support safe medicines management, including reviewing treatment efficacy, side effects, interactions, and risk factors.

  • Liaise closely with partner pharmacies to ensure accurate dispensing and timely delivery of medications.
  • Maintain accurate, contemporaneous electronic patient health records following consultations and prescribing decisions.
  • Act as a Level 3 Safeguarding practitioner, identifying, assessing, and escalating safeguarding concerns appropriately.
  • Implement prescribing changes in response to MHRA alerts, product withdrawals, and other relevant national or local guidance.
  • Participate in clinical audits to ensure safety, compliance, and continuous quality improvement.
  • Work alongside governance and operational teams to ensure compliance with CQC standards where medicines management is involved.
  • Contribute to service efficiency and sustainability through monitoring, audit, and quality improvement initiatives.
