Under general supervision, performs the full scope of duties associated with professional pharmacy practice. Is empowered to use judgment in the decision-making process within their sphere of activity.

Graduation from an accredited School of Pharmacy (BS or Doctor of Pharmacy) plus current license and registration to practice in New York State. Minimum 3 years experience as a pharmacist in Hospital Pharmacy with accepted standards of practice. PharmD or MS degree and/or completion of Pharmacy Residency Program may be used as a substitute for some experience.

Maintains professional competence through participation in continuing education programs and review of periodical literature. Maintains and provides proof of 15 Continuing Education credits each year.
