Responsibilities

  • Care Management – Receive referrals, conduct comprehensive assessments, develop person-centered plans, coordinate services, maintain regular contact, provide home visits, and travel as required.
  • Documentation – Complete all required documentation in a clear, concise, and timely fashion; maintain up-to-date assessments and care plans in the electronic record.
  • Collaboration – Work as a member of the multidisciplinary Care Management team, facilitate meetings with members, providers, and family, and participate in inter-agency teams.
  • Communication – Present professionally, provide feedback to providers, engage members, and deliver program information as needed.

Minimum Requirements

  • Master’s degree in a Human Services field and 1 year of experience with serious mental illness, developmental disabilities, or substance abuse; OR Bachelor’s degree and 2 years of such experience; OR credentialed Alcoholism and Substance Abuse Counselor (CASAC) with 2 years of experience.
  • Demonstrated ability to respect diversity and maintain confidentiality.
  • Strong teamwork, communication, organizational, and computer skills.
  • NY-state driver’s license and reliable transportation for travel to member homes.

Preferred Qualifications

  • Previous experience as a Health Home Care Manager.

Monroe Plan for Medical Care is an Equal Opportunity Employer. The minimum and maximum annual salary for this position is $46,948 – $57,380. In addition to salary, Monroe Plan offers a comprehensive benefits package and non-monetary perks. Candidate must be willing to travel throughout the Rochester and surrounding area and have experience working with adults.
