We are looking for a part-time Nurse Care Coordinator to join our AWC Division for 30 hours per week. This is a varied and rewarding role focused on coordinating personalised care and support planning, ensuring patients receive seamless, holistic care across primary care and wider health and social care services.

Main duties of the job:

This is a dynamic, patient-focused role that ensures health and care planning is timely, efficient, and centred around individual patient needs. You will bring together information relating to a patient's identified care and support requirements and work collaboratively to explore and coordinate appropriate options within a single personalised care and support plan.

The role requires flexibility, innovation, and commitment and is not limited to standard working patterns. You will work resourcefully as part of a multidisciplinary team to ensure tasks are completed effectively and patient care remains seamless.

  • Experience undertaking quality improvement activity

Knowledge:
  • Level 2 qualification in Maths and English
  • Understanding of health and social care processes
  • Knowledge of medical terminology
  • Understanding of social determinants of health
  • High levels of health literacy
  • Excellent knowledge of Microsoft Office
  • Educated to Level 3 in a relevant subject or working towards
  • Knowledge of digital tools supporting health and wellbeing
  • Understanding of current NHS challenges
  • Knowledge of disease prevention (primary, secondary, and tertiary)
  • Aware of local services and community resources
