Ready to make a real impact in Primary Care? We are seeking a motivated and experienced Advanced Clinical Practitioner (ACP) to join the Hull Division of Modality Partnership. Main duties of the job
This advanced clinical role requires a high degree of autonomy, professional judgement, and leadership. You will carry out comprehensive assessments, diagnose and manage conditions, and develop evidence‑based, personalised care plans in partnership with patients and multidisciplinary teams. Working autonomously, you will provide safe, timely clinical decision‑making for patients with acute, long‑term, and complex needs, applying advanced clinical and prescribing skills where qualified. Beyond direct patient care, you will contribute to service development, quality improvement, and innovative ways of working, collaborating with divisional and national teams to enhance processes and support sustainable service transformation. Flexibility, initiative, and commitment are essential to maintaining high standards of care, governance, and operational delivery.

Job responsibilities
Pre-employment Requirements
For this role, you will be required to provide evidence of full Hepatitis B vaccination including three doses with titre level and or booster, along with MMR and Varicella vaccination status. Support will be provided where required.

Qualifications
- Registered first‑level general nurse.
- Recognised degree qualification in autonomous practice.
- Minimum of five years post‑registration experience.
- Minimum of five years recent experience in primary and community nursing.
- Mentor or teaching qualification.
- Clinical supervision training and experience.
- Extended or supplementary prescribing qualification.
- Masters in Advanced Practice and be an independent prescriber.
- MSc or equivalent.
- Experience of developing and maintaining effective partnerships with external organisations.
Knowledge
- Understanding of the local healthcare system, including its priorities and challenges.
- Understanding of national health and social care policy and emerging strategies.
- Understanding of the evolving Advanced Clinical Practitioner role.
- Advanced clinical practice in managing minor illness and acute presentations.
- Experience managing patients with complex and multiple needs.
- Understanding of clinical governance within Primary Care.
- Knowledge of Patient Group Directions and associated policies.
- Understanding of the wider health economy and accountability frameworks.
