The predicted salary is between 37338 - 44962 Β£ per year.

We are seeking a skilled, compassionate, and experienced Diabetes Nurse Specialist to join Enki Medical Centre on a part-time basis (25-30 hours per week). This is far more than a traditional nursing role; you will play a vital part in the demonstration of specialist competence, innovation and clinical leadership in the assessment, intervention in and delivery of general practice nursing in all contexts appropriate to individuals' needs, across all ages for the general practice population. They will specialise in Diabetes and will work to help and support patients who are diagnosed with Diabetes.

Main duties of the job

This role focuses on the delivery of high-quality nursing care to patients to assess, review and support patients who are diabetic. You will use your clinical expertise and professional judgement to assess, plan, develop, implement, and evaluate treatment programmes that promote health and well-being. Working innovatively and flexibly, you will contribute to service improvement and support the effective delivery of care across the practice. The role requires commitment, adaptability, and the ability to work resourcefully as part of a multidisciplinary team, ensuring safe, effective and timely care in line with our CARE values.

Pre-employment Requirements

  • Vaccinations: Mandatory vaccination evidence is required for this role, including Hepatitis B, MMR, and Varicella.
  • Right to work: Proof of eligibility to work in the UK is required at interview stage.
  • References: References must be obtained prior to employment, including one from your current or most recent employer.
  • Employment history: Any employment gaps of six weeks or more must be declared.

Qualifications

  • Registered Nurse qualification with a minimum of two years post-registration experience.
  • At least one year of experience in General Practice or community nursing.
  • Experience implementing clinical guidelines and protocols.
  • Experience in audit activity and quality improvement initiatives.
  • Clinical supervision training and experience.

Knowledge

  • Speciality in Diabetes.
  • Knowledge of diagnosis and management of patients with chronic conditions.
  • Knowledge of accountability of own role and other roles in primary care.
  • Knowledge of health promotion strategies.
  • Knowledge of local and national health policy.
  • Wider health economy awareness.
  • Knowledge of clinical governance issues in primary care.
  • Knowledge of patient group directions and associated policy.
  • Knowledge of local and national guidance and policy relating to all relevant areas of own practice.
